A custom solution for collecting medical questionnaires

This custom solution digitalizes the process of collecting medical questionnaires by enabling parents to submit forms online through a secure platform. Staff can access the submitted data instantly, eliminating manual input.

The client

Centrum voor Leerlingenbegeleiding (CLB) is an institution that partners with every accredited school in Flanders (Belgium) to support students in terms of learning and studying, educational career guidance, preventive health care and psychosocial well-being.

Client Challenge

Each year CLBs collect medical questionnaires for children enrolled in schools that are within their jurisdiction. The process was done manually: CLB workers would bring medical forms to schools and later collect them once the children’s parents had filled them in. After that, the CLB workers had to manually input the information into their system which was very time-consuming and inefficient.

The solution

The solution is a completely custom platform called CLB Medical Questionnaires, accessible to parents when they need to fill in a form for their children.

Parents access the platform and verify their identity through an SSO integration with eID and by entering their child’s social security identification number, allowing the system to confirm the child’s school enrollment.

Once a questionnaire is submitted, CLB employees receive a notification and can directly access the completed form, eliminating the need for manual data entry.

Tech solution

We opted for Spring Boot and Angular. To support the required multiple layers of security, we implemented Spring Security.

AWS cloud services ensured high availability, reduced infrastructure costs, and accelerated platform development.
